• Provides the ability to transfer and continue health insurance coverage for millions of American workers and their families when they change or lose their jobs;
• Reduces health care fraud and abuse;
• Mandates industry-wide standards for health care information on electronic billing and other processes; and
• Requires the protection and confidential handling of protected health information

Yes, the Texas Health and Safety Code, Chapter 611 allows the provider to provide a labor fee for records.  Additionally, The Privacy Rule permits a covered entity to impose a reasonable, cost-based fee if the individual requests a copy of the Private Health Information.  The fee may consist of (1) a labor for copying the PHI requested by the individual, whether in paper or electronic form; (2) supplies for creating the paper copy or electronic media (e.g., CD or USB drive) if the individual requests that the electronic copy be provided on portable media; (3) postage, when the individual requests that the copy, or the summary or explanation, be mailed; and (4) preparation of an explanation or summary of the PHI, if agreed to by the individual.

To cancel an appointment, please call (972) 895-2502 as soon as possible after you become aware that you are unable to attend your appointment. Please note that you must provide a 72 hour cancellation notice, in order to not be charged your full session rate.

Yes, we offer limited evening appointments. Please check for availability. We offer Saturday appointments upon request.

A parenting facilitator helps parents to establish and maintain a co-parenting relationship. The main focus of parenting facilitation is to reduce parental conflict and keep children out of the middle. Parenting facilitation is not confidential.
